All-On-4™- an antiquated treatment

The All-On-4™ implant treatment was an excellent treatment option for patients who required most or all of their missing teeth to be replaced. This technique involved the placement of as few as 4 dental implants at appropriate mechanical inclinations to maximize fixed support for a full arch of teeth. It dramatically improved a patient’s quality of life by avoiding the need for an uncomfortable denture.

All-On-4™ implants was a much improved treatment alternative for patients who had been stuck in the past with dentures due to finances or anatomical reasons. Traditionally, the placement of up to 8-10 implants per jaw was necessary to restore these patients with a fixed bridge instead of a denture. To place this large number of implants, patients often had to have additional extensive bone grafting procedures which further added to the high cost of treatment. With the All-On-4™ concept, the placement of 2 back implants at up to a 45° angulation along with conventional placement of usually 2 implants in the front provided enough support for a fixed bridge while avoiding the need for extensive bone grafting.

 

BUYER BEWARE!

While the surgical concepts used for the All-On-4™ treatment are still useful today, the traditional methods of prosthetically restoring it are riddled with problems. Specifically, the vast majority of office offering an All-On-4™ style of treatment provide patients with hybrid bridges as the final prosthesis. These bridges have plastic teeth glued onto a metal bar. The bond of these teeth to the metal bar is unpredictable in the longterm and the plastic teeth themselves are prone to fracture after prolonged wear. Often these style bridges fail because of teeth popping or breaking off. The result is an All-On-4™ prosthesis that is very high-maintenance, requiring multiple repairs and possibly the need for frequent replacement all at an added expense to the patient. This is very frustrating for patients because after spending significant finances on the All-On-4™ style treatment they are required to invest more money in the continued maintenance of their bridges. So just because an office advertises they offer an All-On-4™ style treatment, make sure to ask what type of bridge will be designed and how long can one expect it to be in service.
