Office fee schedule subject to change. This program is a discount plan, not a dental insurance plan. If you have any questions or are unsure about what is included please call our office directly.   • Eligibility begins the date of sign-up and the membership fee is collected in-full annually.
    • iD Value Membership Plan $936/ Annually
    • iD Platinum Membership Plan $1188/ Annually
    • iD Perio Membership Plan $1788/ Annually
  • Patient portion of the bill is due at the time of service.
  • Cannot be used in conjunction with another dental discount plan, any other promotion, or dental insurance accepted in our office.
  • No refunds
  • No membership cards will be given. Your plan’s effective eligibility date will be on file with our office. 
  • Membership plan cannot be used at any other office other than integratedDENTAL and its providers.
  • Membership plan cannot be used for a referral to specialists or for hospital care.
  • Membership plan cannot be used for costs of dental care which is covered under medical or automobile medical insurance.
  • Membership plan cannot be used for injuries covered under workers’ compensation claims.
  • Membership plan cannot be used for treatment for which, in the sole opinion of the providers, lies outside the realm of their capability.
  • Maximum annual discount on all services is $5,000.
  • Excludes products for purchase, fluoride treatments, and sealants.
